A campus activity form should be submitted to the Facilities & Planning department to facilitate any campus activities scheduled to take place in any of the Lawson State’s auditoriums, the gymnasium, or the cafeteria.

AFTER the proper permission to use a campus facility is granted, THEN complete this form in it’s entirety to request Audio/Visual Assistance within 5 days of the event

This form should be completed for the following:

1. CAMPUS/CLASSROOM ACTVITY/SET-UP: Complete the AV Request in its entirety and submit it in no less than five working days prior to the planned event.

2. AV EQUIPMENT MALFUNCTION: To report equipment malfunction, microphone not working, batteries needed, projector bulb blown, etc.

Audio visual assistance should be requested no less than five business days prior to the event date. Requests submitted less than five days notice may be declined due to previously scheduled events. Lawson State Community College Administrators, Faculty, and Staff needing assistance may schedule appointments for assistance with by calling 205-929-2068.
